Treatment contract and declaration of consent UROMED Competence Center Urology

Unless otherwise agreed in writing, the following conditions apply to the use of our medical and non-medical services in our practice.
1. You must present your e-card or an official photo ID every time you make use of social insurance benefits and every time you visit our practice. If you have forgotten your e-card or ID card or are unable to present it, we will unfortunately not be able to accept you as a health insurance patient.
2. If you have seen another urology specialist during the quarter, we will not be able to accept you as a patient until the next quarter.
3. A single treatment contract is concluded when you register at the surgery or are treated by our doctors.
4. If services are required that are not paid for by your health insurance company, the cost reimbursements, fees or other charges incurred must be paid upon receipt of the invoice.
5. Exclusion of insurance cover: Certain services provided in our practice, in particular private consultations, sexual medical consultations, spermiograms, urodynamics or vasectomies are private services which are not reimbursed by your health insurance company. Even if you have private health insurance, there is no guarantee for the scope and amount of reimbursement by your private health insurance.
6. We accept no liability for any items you bring into our surgery.
7. Appointments: It is important to us that all patients in our practice have the shortest possible waiting times. For this reason, it is necessary that you keep the agreed appointments or cancel them in good time. You can cancel appointments 24 hours before the start of the appointment in writing at (storno@uromed.at). If you do not cancel in good time, we may charge a flat-rate fee of up to 100&euros. If it is not possible to cancel an appointment in writing, we will also accept cancellations by telephone. However, for your own safety, we recommend that you cancel in writing. If you do not cancel a private appointment (no health insurance services, as stated above) up to 24 hours in advance, we are entitled to charge a fee of 100&euros. In the case of a planned vasectomy, you are obliged to cancel at least one week before the appointment; if this deadline is not met, we are entitled to charge an expense allowance of 900&euros. In any case, if you cancel a vasectomy, you will be charged 150&euros for the consultation. If the appointment for a vasectomy is postponed more than twice, we are also entitled to charge 150€ as compensation for expenses.
8. Requesting findings and making appointments: You agree that if you use cgm-life.at to make appointments, you also agree that the required data may be stored and used for the purpose of preparing and making appointments. You can revoke this consent at any time.
9. You agree to the electronic processing of your data (appointment scheduling, management and administration, including health data) by Dr. Mehmet Özsoy and Uromed & ndash; Kompetenzzentrum Urologie. You also agree that we may transfer your data to other doctors or medical institutions whose treatment you are undergoing or will undergo, in particular for the purpose of preparing findings. This consent can be revoked in writing at any time.

Değerli hastamız,
Muayenehanemizde sunduğumuz tıbbi ve tıbbi olmayan hizmetlerimizde aşağıdaki Sartlar geçerlidir: Bu şartnamenin dışına sadece yazılı bir ek şartname ile çıkılabilir.
1. Sağlık sigortanızın geçerli olduğu hizmetlerimizde ve her muayenehanemize geldiğinizde sahsınıza ait olan E-card veya kimliğinizin yanınızda bulunması gerekmektedir. E-Card veya kimliğinizin yanınızda olmaması durumunda sağlık sigortası kayıt girişini yapamamaktayız.
2. Eğer içerisinde bulunduğumuz ceyrekyil (quarter) içerisinde başka bir üroloji uzmanına danıştıysanız ancak bir sonraki ceyrekyil (quarter) içerisinde muayenehanemizde hastamız olarak kayıt yaptırabilmektesiniz.
3. Muayenehanemize kaydınız yaptırdığınız andan itibaren veya doktorlarımız tarafından tedaviye başlanıldığı andan itibaren bu tedavi sözleşmesi yürürlüğe girer.
4. Eğer yapılan muayeneler sosyal sağlık sigortanız tarafından karşılanmıyorsa bu muayenelerin bedeli size faturalanacaktır ve sizin tarafınızdan hemen ödenmesi gerekmektedir.
5. Muayenehanemizde verdiğimiz hizmetlerden bazıları; örneğin cinsel sağlık danışmanlığı, özel muayene, Spermiogram, Vasektomi ve Ürodinami gibi hizmetlerin özel muayene (PRIVATE SERVICES) baslığı altında değerlendirilmesi sebebiyle sağlık sigortası tarafından karşılanmamaktadırı. Bu hizmetler için ödediğiniz bedeli sosyal sağlık sigortanızdan geri almanız mümkün değildir. Özel sigortanız olsa bile bu ücretlerin özel sigortanız tarafından karşılanacağına dair size bir garanti veremeyiz.
6. Muayenehanemize getirdiğiniz şahsi eşyalarınızın sorumluluğu size aittir. UROMED'in şahsi eşyaların güvenliğiyle ilgili herhangi bir yükümlülüğü yoktur.
7. Randevular: Muayenehanemize gelen tüm hastalarımızın mümkün olduğunca kısa bir bekleme süresi sonrasında tedavi görmeleri bizim için büyük önem taşımaktadır. Bu nedenle muayenehanemizden aldığınız randevunuza zamanında gelmenizi veya gelemeyeceğinizi bize önceden iletmenizi sizden önemle rica ediyoruz. Randevunuzu iptal etmek istediğinizde en geç 24 saat önce yazılı bir şekilde (tercihen email adresimize: storno@uromed.at) randevu iptalinizi bildirmeniz gerekmektedir. Eğer yazılı bir şekilde bildirmeniz mümkün değilse randevülerinizi bize telefon ederek iptal ettirebilirsiniz, fakat hastalarımıza daha garantili olan yazılı randevu iptalini önermekteyiz. İptal edilmeyen randevular için 100,00€ kadar hizmet karşılık bedeli ödemeniz gerekmektedir.
8. UROMED'den alinmiş olan özel hizmet randevularının (PRIVATE SERVICES) en geç 24 saat önce yazılı olarak iptal edilmesi gerekmektedir. İptal isteğinizi bize zamanında bildirmemeniz halinde hizmet bedeli olarak sizden 100,00€ ücret talep etme hakkına sahip bulunmaktayız.
9. Vasektomi randevularının ameliyat tarihinden en geç bir hafta önce iptal edilmesi gerekmektedir. 1 hafta öncesine kadar iptal etmediğiniz vasektomi randevuları için 650,00€ hizmet ücreti ödemeniz gerekmektedir. Yeterli süre öncesinde Vasektomi randevunuzu iptal etmiş olsanız bile ilk yapılan bilgilendirme görüşmesi bedeli olarak 150,00€ ücret alınmaktadır. Eğer Vasektomi randevunuzu ikinci defa ertelerseniz ekstra hizmet ücreti 150,00€ ödemeniz gerekmektedir.
